Advance Care Plan
An Advance Care Plan records the medical care you would want if you could not speak for yourself. Your family and your doctors use it as a guide to your wishes.
What it covers
Your plan sets out your preferences on matters such as the extent of treatment and pain relief.
It also lets you name up to two Nominated Healthcare Spokespersons. They tell your doctors what you would want if you cannot say so yourself.
How it differs from an LPA and an AMD
A Lasting Power of Attorney is a legal document. The people you appoint, called donees, can make decisions on your personal welfare and on your property and affairs if you lose mental capacity.
An Advance Medical Directive is also a legal document. It tells your doctor that you do not want extraordinary life-sustaining treatment if you become terminally ill and unconscious.
An Advance Care Plan is not a legal document. It gives your family and your healthcare team a guide to your wishes.
My Legacy encourages you to choose the same person as your Lasting Power of Attorney donee for personal welfare and as your Nominated Healthcare Spokesperson. If they are different people, the donee remains the legal decision-maker.
How to make one
- Prepare your plan online using the myACP service.
- Each Nominated Healthcare Spokesperson receives an SMS and accepts the appointment on My Legacy. Your plan is registered once they have all accepted.
- Share the plan with your family and your doctors.
You can use myACP if you are 21 or over and do not have a serious illness. If you have a serious illness, make your plan with an Advance Care Planning facilitator instead.
You can also meet a facilitator if you prefer, online or in person.
The Government's Plan Your Legacy website estimates that starting a plan takes about 30 minutes.
